I love this educational post/ article by @joleneewrites in Allure (my edit). Nonsurgical rhinoplasty is not as easy as it looks— and neither is any injectable procedure for that matter. The best injectors are the least cavalier. For me, experience in the OR gives me three-dimensional understanding of facial anatomy that helps me perform injectable procedures safely. Even when a procedure seems like a quick and easy push of a syringe, there is often a decade or more of education and training behind its successful completion. In addition, while injectables are overall safe, risks can be serious— including tissue necrosis and blindness. I am of two minds about injectables. There is a part of me, like most board-certified plastic and facial plastic surgeons, that feels that we are uniquely qualified to understand facial anatomy and should be everyone’s injector. However, I’m not sure this is practical for the population, and over time my perspective has broadened and become more nuanced. There are talented providers with many different backgrounds. Moreover, skills are teachable, and indeed this is the basis for medical education. So where do we go from here in medical aesthetics? Who should do your botox? I think the answer is three-fold: 1️⃣ providers should honestly self-assess and perform only the procedures they can do best. If you can’t draw the vasculature, musculature, and innervation of the face, don’t stick a needle into it. For some high-risk and high-complexity areas like the eyes, nose, and glabella, an even higher standard should be respected. 2️⃣ Understand the management of complications, and always have a “filler crash cart.” 3️⃣ Share knowledge and always learn. This is a closer look at the nonsurgical rhinoplasty from my previous post. By making many tiny changes to the nose I was able to reduce the appearance of the dorsal hump, improve the nasolabial angle, raise the radix height, and make the tip appear more delicate. This is a complex and nuanced injectable procedure that requires thorough anatomic knowledge that is best understood in the context of surgical rhinoplasty. Still, it is a teachable technique, and there are ways to make it safer for everyone.
