A subtle nonsurgical rhinoplasty transformation for my beautiful patient. Here I focused on lifting the nasal tip, straightening the dorsum, and making the nose appear more delicate. WHAT IS FILLER? Most injectable fillers are made from a naturally occurring sugar molecule derived from the body’s own collagen. It looks and feels like a clear hair gel or jelly, and it can vary in density, viscosity and G-prime (or lift-ability) depending on the specific subtype. Fillers can last months to years and can be used in a variety of ways. They fade away gradually as the body breaks them down— like the process of an ice cube melting. Surgical or nonsurgical? This beautiful patient is an excellent example of how injectable tear trough augmentation can solve some but not all of the concerns of the undereye area. The concept of tear trough filler is that we are softening the undereye hollows (for me, by layering filler above and below the orbicularis oculi muscle and on the periosteum of the bone in a sandwich 🥪 technique). This can camouflage the appearance of puffiness and excess skin to some extent, as seen here— but blepharoplasty surgery remains the gold standard approach. If surgery is not on your radar screen for whatever reason, a nonsurgical improvement may be possible. During my consultations for this procedure, I make a point of discussing when surgery would provide a more robust result.
