Softening deep undereye hollows with tear trough augmentation 💉💉💉 This young patient has beautiful large eyes but also has a large bony orbit. Normal fat loss around the eyes created a hollow and dark look that she felt made her look tired. I used hyaluronic acid dermal filler in a sandwich 🥪🥪🥪 technique (layered on the periosteum, below the orbicularis oculi musculature, and above the orbicularis oculi) to give her this improvement. 💉💉💉 Tear trough augmentation is one of my favorite techniques, but it bears repeating that periorbital anatomy is quite complex. Because the vasculature is very involved, about 1 in 20 patients will get a bruise. Because fillers draw in water molecules, most patients will experience transient swelling or puffiness in this area. Because orbital size and anatomy vary greatly, some patients will get a full correction with just one session, and others will require 4 or more for a complete augmentation. Because metabolic rates vary, some patients will have a result that lasts for a few months, and others will have an excellent result after 3-4 years. In many if not most cases, surgical blepharoplasty is the gold standard approach. 💉💉💉 My approach to the tear troughs has been discussed in the @nytimes, presented at the @plasticsurgeryasps national meeting, and taught to 250+ injectors around the world in my Beauty Metrics Master Classes, but injectable fillers are still just injectable fillers. We can accomplish a lot with them— but not everything. Aesthetic misadventures and odd facial conformations occur when people try to accomplish too much with this inherently limited modality. 💉💉💉 I enjoy being able to offer my patients the entire spectrum from skin care to lasers to injectables to surgery, but in all cases, it is important to understand what is realistic and what is not in terms of outcomes. 💉💉💉 .
